I have an LMS. I need a summary on a single page of the exam answers. NOTICE: it is NOT a dynamic template. I do NOT need to collect the answers from the database. It can be a template with fixed data of the answers.

It is only for 2 courses. I need someone with LMS experience. Date variables: date, applicant, applicant name. Fixed date: instructor, instructor signature IN JPG, No digital signature required.
